🤝 Homelessness to Home Healthcare Network Forum

Pathways to health equity for homeless people and families

Guest speakers including Dr Amanda Stafford (WA), Clinical Lead of the Royal Perth Hospital Homeless Team, will explore the purposeful interventions, underlying structures and supporting mechanisms which would allow an integrated health and housing response to improving health outcomes for individual and families who are experiencing homelessness and vulnerably housed, creating benefits to our systems and community.

💡 Hospital Discharge into Homelessness Forum

The Logan region is home to many rough sleepers who are experiencing severe health and mental health conditions. The interface between the housing and health systems can be challenging to navigate, with many rough sleepers reporting increasing presentations to emergency departments and discharges from hospital into homelessness. This forum seeks to elevate awareness of the systemic health and mental health issues faced by rough sleepers in Logan. It aims to build an alliance and consensus on priority areas and develop actionable steps to address these critical issues.
